Background: Adverse drug event (ADE) errors are common and costly in health care systems across the world. Medication reconciliation is a means to decrease these medication-related injuries and increase quality of care. Research has shown that medication reconciliation accuracy and efficiency improved when pharmacists are directly involved in the process. Objective: We review studies examining how pharmacists impact the medication reconciliation process and we discuss pharmacists' future roles during the medication reconciliation process and then barriers pharmacy staff may face during this critical process. Methods: A comprehensive literature search from MEDLINE and manual searching of bibliographies was performed for the time period January 2012 through November 2018. Conclusion: Although the issue of rising costs and injury due to medication errors in our health care system are not solvable via medication reconciliation alone, it is the first and perhaps most critical piece of the medication management puzzle. As such, numerous organizations have called for pharmacists to expand their roles in the medication reconciliation process due to their expertise in medication management.

A catalytic system for the chlorination of alcohols under Appel conditions was developed. Benzotrichloride is used as a cheap and readily available chlorinating agent in combination with trioctylphosphane as the catalyst and phenylsilane as the terminal reductant. The reaction has several advantages over other variants of the Appel reaction, e.g., no additional solvent is required and the phosphane reagent is used only in catalytic amounts. In total, 27 different primary, secondary, and tertiary alkyl chlorides were synthesized in yields up to 95%. Under optimized conditions, it was also possible to convert epoxides and an oxetane to the dichlorinated products.

This paper studies integral schemes X defined over a field of characteristic which admit a nontrivial or action. In particular, the quotient map is investigated and structure theorems for it are obtained. Moreover, information on local properties of the quotient Y, like singularities and local Picard groups as well as an adjunction formula for the quotient map are also obtained.

The present paper deals with the Stancu-type generalization of (p, q)-Baskakov-Durrmeyer operators. We investigate local approximation, weighted approximation properties of new operators and present the rate of convergence by means of suitable modulus of continuity. At the end of the paper, we introduce a new modification of (p, q)-Baskakov-Durrmeyer-Stancu operators with King approach.
