  • Comparison between major and trace element concentrations in garnet performed by EPMA and micro-PIXE techniques

    This paper deals with the application of the electron and proton microprobe (EPMA and micro-PIXE) in order to determine major and trace elements in metamorphic garnet samples. The selected garnet samples come from a metapelitic rock belonging to the tectonic unit of Monte Rosa Nappe (Italy). Quantitative spot analysis profiles and compositional X-ray maps of both major and trace (yttrium) elements are reported. Major elements show a smoothed and continuous compositional zoning characterized by concentric variations from core to rim. Yttrium displays a strong enrichment in the core and a flat pattern at the rim. No correlation is shown between major and yttrium. distribution. Only a rough correlation may be supposed with manganese. A good agreement was found between EPMA and micro-PIXE yttrium. data, for values higher than 80-100 ppm. For lower values micro-PIXE technique is strongly recommended. The combination of X-ray two-dimensional maps and quantitative microanalyses allowed to evaluate the distribution of major and trace elements in a petrologically significative rock-forming mineral, to define the type of the chemical zoning and finally to attest difference in diffusivity between major and trace elements. Because of the slow diffusivity of yttrium, its quantitative determination in garnets is fundamental to reconstruct the temperature path suffered by polymetamorphic garnet-bearing rocks.   • Surgical treatment of arrhythmias related to congenital heart diseases

    Background. Supraventricular arrhythmias complicate operated and unoperated congenital heart disease, especially when atrial dilatation coexists. Methods. We evaluated the results of intraoperative ablation in a group of 23 patients with chronic supraventricular tachyarrhythmias (mostly intraatrial reentry) that were unresponsive to conventional medical therapy. All procedures were performed consecutively between September 1999 and November 2001. Ablation was done during redo operations (Fontan conversion to total cavopulmonary connection in 16 patients) in 18 patients and during primary surgical correction in 5 patients. The mean age at operation was 25 +/- 12 years (2 to 50 years). Cryoablation was done in 10 patients and radiofrequency ablation in 13 patients. Nineteen patients had ablation in one atrium and 4 had ablation in both atria. A generous atrial reduction was always performed at the end of the operation. Results. The operative mortality rate was 13% (3 patients) from causes unrelated to ablation. In 20 survivors, the ablation was effective immediately. Eight patients required a permanent pacemaker. During a mean follow-up of 22 4 months, atrial arrhythmias recurred in 25% (5 patients) and were controlled with medical therapy, whereas 1 patient required pacemaker implantation. Conclusions. Intraoperative treatment of unresponsive atrial tachyarrhythmias associated with operated or unoperated congenital heart disease is feasible and the midterm results are encouraging.   • Continuous vs. tapering application of the potent topical corticosteroid mometasone furoate in the treatment of vulvar lichen sclerosus: results of a randomized trial.

    BACKGROUND: Topical corticosteroids are the first-line treatment for vulvar lichen sclerosus (VLS). However, evidence on the most appropriate treatment regimen is lacking.; OBJECTIVES: To compare the effectiveness and tolerability of tapering vs. continuous application of mometasone furoate (MMF) 0=C2=B71% ointment in the treatment of active VLS.; METHODS: Sixty-four patients with VLS who enrolled in a 12-week active treatment phase were randomized to apply MMF either (i) once daily for 5days per week for 4weeks, then on alternate days for 4weeks, then twice weekly for 4weeks (group A) or (ii) for five consecutive days per week for the entire treatment duration (group B). The efficacy parameters were the response rate, the proportion of patients achieving an improvement from baseline of =E2=89=A575% in subjective and objective scores, and the mean reduction in subjective and objective scores.; RESULTS: By the end of the active treatment phase, 27 patients (84%) were considered to be responders in group A, and 25 patients (78%) in group B; 69% and 47% of patients in group A and 62% and 28% in group B achieved =E2=89=A575% improvement in subjective and objective scores, respectively. The decreases in mean symptom and sign scores were significant compared with baseline with both regimens. No significant differences were found in any of the assessed efficacy end points between the two treatment protocols. Both regimens were well tolerated.; CONCLUSIONS: Both tapering and continuous application of MMF showed similar efficacy and tolerability in the treatment of active VLS, without any difference in patient adherence to therapy. =C2=A9 2015 British Association of Dermatologists.

  • Cutaneous infection caused by Salmonella typhi

    Salmonella typhi is a flagellated, gram-negative bacillus belonging to the family Enterobacteriaceae responsible for typhoid fever, which is a prolonged bacteraemic, systemic illness with minimal, at least initially, diarrhoea. Localized infection is a not uncommon complication of salmonella septicaemia, particularly occurring in immunocompromised patients. However, salmonella localization to the skin presenting as cutaneous ulceration is regarded as a rare event. We report a unique case of such a presentation as the sole clinical manifestation of infection with S. typhi and highlight the possible salmonella aetiology for unusual cutaneous lesions in individuals coming from endemic areas.

  • Combined micro-PIXE facility and monochromatic cathodoluminescence spectroscopy applied to colored minerals of natural stones: an example from amazonite

    Quantitative micro-PIXE and cathodoluminescence spectra obtained for amazonite crystals from Malawi and Brazil granitoids, in order to check the trace elements presumably responsible for their color. These granitoids, used as Natural Stones for inside coverings and furnishings, are precious and rare, and owe their preciousness to the color of the rock-forming aquamarine amazonite. The trace elements investigated by means of micro-PIXE show the occurrence of Fe, Rb, Pb and Ga at concentrations above the minimum detection limit. In particular, the Malawi amazonite has a higher Pb content than the Brazil amazonite, but for Pb content both samples show a nearly flat pattern profile along the whole crystals. The cathodoluminescence spectra in both samples display three main peaks at 284, 558 and 680 nm which may be attributed to the presence of Pb2+, Mn2+ and Fe3+, respectively. As no Mn was detected by PIXE we suggest that its occurrence is below the PIXE detection limit. Micro-PIXE analysis and cathodoluminescence spectra of most common and uncolored K-feldspars do not reveal the presence of lead; therefore we support the hypothesis of Pb as the chromophore source.   • First randomized trial on clobetasol propionate and mometasone furoate in the treatment of vulvar lichen sclerosus: results of efficacy and tolerability.

    BACKGROUND: A 3-month topical application of clobetasol propionate (CP) represents the recommended and accepted first-line treatment for vulvar lichen sclerosus (VLS); however, to date, no randomized controlled trials have compared the efficacy and safety of CP with other topical corticosteroids.; OBJECTIVE: To compare the effectiveness and tolerability of two different topical corticosteroids, CP 0=C2=B705% ointment and mometasone furoate (MMF) 0=C2=B71% ointment, in the treatment of VLS.; METHODS: Fifty-four patients with VLS were enrolled in a 12-week active treatment phase (ATP) and randomized to apply either CP or MMF in a tapering regimen. The main efficacy parameters were the response rate, the rate of patients achieving an improvement from baseline of =E2=89=A575% in the subjective and objective scores, and the mean reduction in subjective and objective scores throughout the treatment.; RESULTS: By the end of the 12-week ATP, 24 (89%) patients were considered to be responders among the CP patients and 24 (89%) among the MMF patients; 59% and 37% of patients in the CP group and 67% and 48% in the MMF group achieved an improvement of at least 75% in subjective and objective scores, respectively. The decrease in mean symptom and sign scores was significant compared with baseline with both treatments. No significant differences were found in any of the assessed efficacy endpoints between CP and MMF. Both treatments were well tolerated.; CONCLUSIONS: Clobetasol propionate and MMF appeared similarly efficacious and well tolerated for the treatment of VLS and both may represent the first-line treatment for the disease. =C2=A9 2014 British Association of Dermatologists.

  • Is body mass index a measure of adiposity in elderly women?

    Objective: To evaluate the accuracy of body mass index (BMI) as a predictor of body fat in elderly women. Research Methods and Procedures: A total of 1423 women aged 67 +/- 5 (mean +/- SD, range: 60 to 88) years were consecutively enrolled into the study. Fat mass (FM) was measured using DXA. Results: BMI explained 72.9% of FM variance (p < 0.0001), with a root mean square en or of estimate (RMSE) of 3.5 kg. After standardization of RMSE on the dependent variable as RMSE%, the prediction error equaled 15%. BMI explained 54.8% of FM% variance (p < 0.0001), with an RMSE of 4.1%, corresponding to an RMSE% of 11%. Discussion: The relatively high RMSE% of the FM and FM%-BMI associations caution against the use of BMI as an adiposity index in individual elderly women. However, an error corresponding to 11% of FM% may be accepted for population studies of body fat in elderly women.

  • Breakdown of the bi-dimensional symmetry in bct Fe layers by epitaxy on Co(1 1 (2)over-bar-0) surface

    The structure of thin Fe overlayers on the Co(1 1 2 0) surface has been investigated by low energy electron diffraction (LEED) and by primary-beam diffraction modulated electron emission (PDMEE), as a function of the Fe coverage (6-50 ML) and of the substrate temperature (between room temperature and 300 degreesC). The structure of the layers was the tetragonally distorted bet modification of the equilibrium bcc phase with the (0 0 1)Fe//(1 1 2 0)Co and [1 1 0]Fe//[1 1 0 0]Co relationships with the Co substrate. The Fe(0 0 1)-Co(1 1 2 0) system exhibits a very large misfit asymmetry in the interface plane. As a consequence, a breakdown of the long-range order occurs in the Fe film along the [1 1 0 0] substrate direction. (C) 2001 Elsevier Science B.V. All rights reserved.
