Despite worldwide and Polish guidelines, recommended vaccinations in preterm children are often performed within a significant delay. It is the result of taking care of security and efficacy, and, furthermore, being afraid of complications. Our idea to start vaccinations according to the recommended schedule in the departments of pathology and intensive care of neonate before discharging children home meets the recommendations of experts, paediatricians and parents.MaterialIn the period between April 2011 and March 2012 there were 103 children born before 32nd Hbd in the Neonatal and Intensive Care Department Medical University of Warsaw. Out of this group, 47 were vaccinated: 46 against pneumoccoci and 34 against DTaP and HIB, respectively.ResultsNeonates vaccinated during the hospitalization constituted 45.63% of all children born before 32nd gestational age. Mean body mass of vaccinated children was 979.56 g (range from 520 to 1600 g). Mean time of vaccination is presented in table 1 and 2. Children born between 24th and 26th gestational age could be immunized on average at 12–14th week of life (175–88th day of life), while neonates born between 27–29th gestational age – on average at 7–9th week of life (69–50th day of life), respectively. Children born between 30–32nd gestational age were vaccinated on average at 6–7th week of life (47–39th day of life).ConclusionsStarting the vaccination in children born before 32nd gestational age during their hospitalization in the department of neonatology before discharging home influences effectively on performing the following immunization according to the schedule and monitoring the child for the next 48–72 hours. Children born before 26th gestational age must not be vaccinated until 12 week of life. Children born between 30-32nd gestational age may be vaccinated with a first dosage of DTaP -HBV-IPV-HIB at the scheduled time (in 8th week of life). Vaccinations in neonates born prematurely are safe and very seldom connected with increased frequency of post-vaccination side-effects.
Singularities in the dark energy late universe are discussed, under the assumption that the Lagrangian contains the Einstein term R plus a modified gravity term of the form R α, where α is a constant. It is found, similarly as in the case of pure Einstein gravity [Brevik and Gorbunova, Gen. Rel. Grav. 37, 2039, 2005], that the fluid can pass from the quintessence region (w > − 1) into the phantom region (w < − 1) as a consequence of a bulk viscosity varying with time. It becomes necessary now, however, to allow for a two-fluid model, since the viscosities for the two components vary differently with time. No scalar fields are needed for the description of the passage through the phantom barrier.
Die gesetzlichen Krankenkassen sind seit Juli 2009 verpflichtet, sogenannte Hausarztmodelle anzubieten. In diesen Modellen wird die Koordinierungsfunktion des Hausarztes gest盲rkt. Bislang ist wenig 眉ber den Nutzerkreis von Hausarztmodellen bekannt. Die vorliegende Arbeit m枚chte diese L眉cke schlie脽en und auf Grundlage des Verhaltensmodells von Andersen Einflussgr枚脽en der Teilnahme an Hausarztmodellen bestimmen. Hierzu werden Daten des Telefonischen Gesundheitssurveys 2006 des Robert Koch-Instituts vorgestellt und Einflussgr枚脽en des Einschreibeverhaltens mithilfe logistischer Regressionsmodelle bestimmt. Wie sich zeigt, nutzten 盲ltere Versicherte, Angeh枚rige der unteren sozialen Schichten, nicht oder nur geringf眉gig Erwerbst盲tige und Personen, die in kleineren Wohnorten leben, besonders h盲ufig das Angebot von Hausarztmodellen. Versicherte mit einem hohen Versorgungsbedarf sind tats盲chlich h盲ufiger in Hausarztmodelle eingeschrieben, auch wenn hierbei noch Verbesserungspotenzial besteht. Es handelt sich bei den Nutzern jedoch in erster Linie um Versicherte, die auch vor Einf眉hrung der Hausarztmodelle h盲ufig einen Hausarzt konsultierten, w盲hrend die Bev枚lkerungsgruppen, die den direkten Weg zum Facharzt w盲hlen, h盲ufig nicht erreicht werden.
Die Deutsche AIDS-Hilfe richtet ihre Arbeit am Ansatz der lebensweisenorientierten strukturellen Pr盲vention und Gesundheitsf枚rderung und an den sich st盲ndig wandelnden (epidemiologischen, medizinischen, sozialen, politischen, kulturellen usw.) Bedingungen aus. Dabei betont sie die gemeinsame Verantwortung der AIDS-Hilfen, anderer Akteure in der Pr盲vention und der Politik, Verwaltung, Wirtschaft und Gesellschaft. Angesichts der ver盲nderten Wahrnehmung der HIV-Erkrankung, der R眉ckkehr des Sexuellen und der zunehmenden Individualisierung und Differenzierung des Risikomanagements sind zur Sicherung des bisher Erreichten und zur Umsetzung substanzieller Verbesserungen unter anderem eine noch gr枚脽ere Lebensweltn盲he und Serviceorientierung von Pr盲vention und Gesundheitsf枚rderung, die verst盲rkte Nutzung aller zur Verf眉gung stehenden Medien des Informationszeitalters, insbesondere des Internets, und klare(re) Botschaften zu neuen Formen des Risikomanagements notwendig.

We report a laser-diode-end-pumped acousto-optical Q-switched double-end diffusion-bonded Nd : YVO4 self-Raman laser at 1176 nm. The maximum average output power at the first-Stokes wavelength of 1176 nm was obtained to be 3.1 W at the incident pump power of 25 W and the repetition rate of 90 kHz, with the corresponding optical conversion efficiency of 12.4%. The shortest pulse width, the maximum pulse energy and the highest peak power were measured to be 5 ns, 42 μJ and 7.5 kW, respectively.
