ZusammenfassungEtwa 2% der Geburten finden geplant oder ungeplant au ss erklinisch statt. Notarzte sollten die Grundlagen der Begleitung einer physiologischen Geburt und die Behandlung eventueller Komplikationen beherrschen. Dafur wird Basiswissen uber den naturlichen Geburtsverlauf, uber Handgriffe, den Einsatz von Uterotonika und Tokolytika sowie uber Ma ss nahmen zur postpartalen Blutstillung benotigt. Die hypertensive Krise, die Eklampsie, aber auch seltenere lebensbedrohliche Notfalle wie eine Lungen- oder Fruchtwasserembolie mussen von Notfallmedizinern unmittelbar erkannt und mit ersten Ma ss nahmen behandelt werden. Der Beitrag soll neben der (un-)geplanten au ss erklinischen physiologischen Geburt die wichtigsten schwangerschaftsbedingten und -assoziierten Notfalle darstellen und notfallmedizinische Handlungsempfehlungen vermitteln. AbstractApproximately 2% of all deliveries in Germany take place either planned or unplanned outside of hospitals. Emergency physicians should master the fundamentals of accompanying a physiological birth and the treatment of possible complications. This necessitates basic knowledge on the course of natural births, manual maneuvers, the use of uterotonic agents and tocolytic drugs as well as measures for postpartum hemostasis. A hypertensive crisis, eclampsia and also rare life-threatening emergencies, such as lung and amniotic fluid embolism must be immediately recognized by the emergency physician and treated with initial measures. This article presents the most important pregnancy-related and associated emergencies in addition to planned and unplanned out of hospital physiological deliveries and gives clear advice about the treatment.
